Card Reload Fee	$2.99	This is a fee that will be deducted from your Card balance every time your Card is reloaded irrespective of the amount. The fee would be identified as “Value Transfer Fee”
Inactivity
Monthly Inactivity Fee	$3.95	This is a fee that will be deducted from your Card balance monthly after each 9-month period of inactivity or non-use of your Card. An imposition of a fee or an adjustment due to an error or reversal for a prior transaction does not constitute activity. The fee would be identified as “Monthly Inactivity Fee”
Replacement
Card Replacement Fee	$5.00	This is a fee that will be deducted from your Card balance if you request a replacement Card prior to the expiration date of your Card. The fee would be identified as “Card Replacement Fee”
Information
Paper Statement Fee	$2.50	You may access your available balance by accessing your Card Account online at www.prepaid-usa.com. Statements in electronic format will be made available at no charge during each month in which a transaction occurs. You will not automatically receive paper statements. You may choose to have a paper statement mailed to you by selecting this option. By selecting this option, a $2.50 charge per paper statement mailed will be applied to your card. There is no charge for online access. The fee would be identified as “Statement Fee”
